His interest in Dental Sleep Medicine evolved from experiences with children and their airway issues.  Crowded teeth can arise from narrow jaws. Narrow jaws can be genetic or be created from outside problems like mouth breathing.  Children who do not breathe well often do not sleep well. It was a natural step to begin looking at adults with the same problems.

As a result, Dr. Fedorowich sets aside a portion of his practice to treat people with breathing and sleep issues.  Over the last 10 years Dr. Fedorowich has completed residencies in Orthodontics, Temporomandibular Joint Dysfunction and has completed numerous advanced courses that deal with Sleep Disorders (snoring and Obstructive Sleep Apnea).

    Biography:

Dr. Irwin graduated from the University of Alabama School of Dentistry in 1968 where he was inducted as a member of Omicron Kappa Upsilon dental honor society and received a scholastic achievement award.  After serving 3 years in the US Army in Belgium he attended the University of Missouri Kansas City for his orthodontic training.  He has been practicing orthodontics in Montgomery since 1973.  In 1992 Dr. Irwin took his first continuing education course on the treatment of snoring and sleep apnea using oral appliances and has been “hooked” ever since.  At that course he began to realize the quality of life is determined by the quality of sleep. Since that time he has made several hundred appliances and takes pride in achieving the maximum benefit for every patient that he treats.

He has a passion for rendering the highest quality of care backed by years of expert clinical experience and the use of only oral appliances which are approved by the FDA

Dr. Irwin has taken many continuing education courses to keep abreast of all the advances being made in the field of dental sleep medicine, and to increase his knowledge and maintain his commitment to excellence in the treatment of patients who suffer from snoring and sleep apnea.

During these years he has worked very closely with the medical doctors in the area who treat patients with sleep disorders.  He firmly believes the treatment of the patients is a “team approach”, and has therefore, maintained an open line of communications with each of them.

Dr. Irwin gets just as much satisfaction from hearing how much he has helped his sleep patients and increased their quality of life as he does seeing the finished smiles on his orthodontic patients.

For the last 4 years Dr. Irwin has spoken to more than 45 civic clubs and organizations in the area on snoring and sleep apnea to increase the public’s awareness of the symptoms, the seriousness of the problem, its medical implications, and the different choices of treatment.

Oral appliances are a great conservative choice for people who snore or have mild to moderate sleep apnea, and also for those with severe sleep apnea who cannot tolerate the CPAP mask.  They can even help in saving a marriage, and oftentimes only a few office appointments are necessary.

    Biography:

Dr. Lawson graduated in 1986 from the University of Manitoba. While general dentistry was his full time service from 1986 until 2005 he developed a keen interest in orthodontics and the dynamics of the growing face. He has completed extensive training all over North America from leaders in Myofunctional Therapy, Functional Jaw Orthopedics, Temporomandibular Joint Dysfunction (TMD), Snoring and Obstructive Sleep Apnea and Orthodontics.

Over the past 24 years Dr Lawson has completed mini-residencies and advanced residencies in Temporomandibular Joint Dysfunction and Sleep Disordered Breathing. He has completed numerous advanced courses that deal with Sleep Disorders (snoring and Obstructive Sleep Apnea). His new facility is a Licensed TMJ and Sleep Therapy Centre, and he works with a group of specialists (ENT’s, Respirologists , and RN’s ) at Rana Respiratory Wellness Centre in Winnipeg offering solutions for snoring and Obstructive Sleep Apnea to patients who have completed Sleep Studies .

In 2005 Dr. Lawson limited his practice to treating these conditions and his new facility utilizes the latest technology to diagnose and treat patients and return them to a state of health where they can enjoy life free from pain and discomfort. He also realizes that the developing face and dentition of children can be positively influenced with similar conservative, non surgical methods which enhance function and facial balance. This will ensure that our children do not suffer from the symptoms associated with unbalanced jaw relationships or an obstructed airway.

The Winnipeg TMJ and Sleep Therapy Centre is dedicated to a comprehensive and differential approach to diagnosis of Temporomandibular Joint Dysfunction and treatment of patients with Sleep Disordered breathing as well as orthodontics and dentofacial orthopedics.

Dr. Melnick incorporates many technologies into his sleep practice, such as the pharyngometer and rhinometer to evaluate a patients airway and help to determine if an oral appliance would be beneficial for a specific individual.  In addition, at-home monitoring is available through the office to assist with the adjustment of an oral appliance.

The Doctor himself is a sleep apnea patient and successfully wears an oral appliance to treat this condition.  He can relate to his patients who also have sleep issues and candidly discusses the condition as well as the different treatment available.

    Biography:

Doctor Morgan began placing oral appliances in 1990 for snoring in his general dental practice. Based on anecdotal evidence of success from his own family and patients, he took his discovery to the physicians at Scripps Clinic Medical Group and home of a world renowned sleep medicine specialty group. That’s where he met Dr Stuart Menn. One of the field’s early leaders in sleep medicine, Menn recognized the potential utility of jaw advancement devices to prevent airway collapse that occurs in sleep apnea and agreed to launch a study on oral appliances and their effects in the sleep laboratory. Prior to this there were no fully controlled trials that supported oral appliance therapy in peer reviewed scientific journals in sleep medicine. In fact, it took more than 5 years and the work of many authors to claim acceptance into the journal SLEEP, recognized still as the prestigious journal of the American Academy of Sleep Medicine.

As a dental clinician, and then not trained in the rigors and protocols of professional scientific investigation, the depth of the contribution that oral appliances would make to the field of obstructive sleep apnea (OSA) today,  could not have been appreciated then . Doctor Morgan has practiced dental sleep medicine (DSM) for more than 12 years, after what is now considered a sentinel work in the science of sleep medicine. However, further opportunities for research to grow the science of this valuable therapy would not present again to Dr Morgan for many years.  Some research was pursued by other investigators after the early discoveries were made, but legitimate endeavors that embraced scientific rigor were few. As a consequence, oral appliance therapy for OSA was stuck in the deep expanse that often separates medical specialties.  The lack of support from medicine for oral appliances squashed significant clinical research and left dentists on their own to promote the field. Dr Morgan was fortunate to have a continued path for development in design and the development of DSM for patients through the progressive thinking of the docs at Scripps Clinic. He would continue to learn more and more over many years about sleep medicine in general and about when to deploy DSM for patients as they were referred to his care.

“Skill, care and judgment” guided Dr Morgan through the treatment of OSA with oral appliances for thousands of patients when referred for a “last chance,” and other therapies had failed. New research from Canada helped to demonstrate the way in which oral appliances prevented airway collapse. Patterns and consistencies emerged with regard to what brought success to patients, and when treatment failed. The fundamental question of how to predict success with oral appliances (and perhaps the most important question for sleep physicians: when to prescribe them), eluded researchers.

A fundamental drive to uncover the reason behind failure in certain cases would bring Dr Morgan to begin his relationship with Dr Philip Westbrook, one of the early giants in sleep medicine. Dr Westbrook recognized that there were many shortfalls in the formal way in which sleep was studied overnight in the sleep laboratory. He also realized the important contribution that dentists could make in the sleep medicine field where CPAP could not meet patients’ needs and expectations. An alliance was formed with Dr Morgan and his company Advanced Brain Monitoring (ABM) to help  uncover the variable effects of oral appliances and, further, to develop novel ways to measure  sleep breathing disorders with portable home monitors.

A 1.5 million dollar grant was awarded by the NIH to Dr Westbrook and  ABM to study the effects of oral appliances, as well as  developing better ways to measure sleep disordered breathing. Doctor Morgan had once again found a path toward research endeavors, and this time he would be able to investigate new and contemporary questions that may lead to better understanding of important questions like: “What patients are best served by oral appliances?” And, “How can treating dentists improve outcomes using the new technology offered by home monitors? Further and perhaps most importantly, “What is the role of dentists in sleep medicine, and can dentists make a meaningful contribution by saving the lives and livelihood of patient affected by OSA?”

Research that began with ABM in 2007 is now concluded and much has been learned already. Of the over 130 patients that completed clinical trials, almost all are completely helped by oral appliances and would prefer them to pressure machines and masks. Both phases of the two part study were multicenter trials and even a novice dental office did very well in providing this care. New insight into differences between types of oral appliances and how sleep study measures can help to steer outcomes were revealed. A critical difference in success is finding jaw positions that are effective and comfortable for the patient. Gender differences and preferred sleep position differences data allows for better predictions about final outcomes with oral appliances. Incredibly, even slight adjustments of jaw protrusion (as small as a quarter of one millimeter) can make significant differences in outcomes. Small amounts of alcohol consumption can skew night-to-night results, as well. Dentists who employ home sleep monitors to guide treatment decisions improve their outcomes by almost 30%, lending credibility to this new technology.

Dr Morgan and his colleagues will continue to unravel more new information about oral appliances using the acquired study data for years to come. A side study by Dr Morgan and colleagues looking at prevalence of OSA in typical suburban dental practices showed that a very high number of patients remain undiagnosed for OSA. The ravages of sleep apnea on the cardiovascular system are very clear and this study alone points toward recognition and referral to qualified sleep specialists as a high priority, and something dentists should be able to do with minimal training.

Dr Morgan has published over 15 abstracts and papers, as well as book chapters on obstructive sleep apnea, its causes and consequences for humans. He lectures nationally on OSA to layperson, as well as dentist and physician groups, on the role of oral appliances in the treatment of snoring and OSA. He is the academic program chair for the Academy of Dental Sleep Medicine, has previously sat on its executive board and remains an ambassador and founding member. His newest research is focused on the relation between OSA and TMJ/headache disorders.

As a new graduate, Dr. Rondeau attended numerous orthodontic seminars across North America and found inconsistent philosophies being taught on the subject of orthodontics. After attending many of these lectures, he developed his own two phase treatment philosophy and cephalometric analysis.

With the sponsorship of a dental lab, Dr. Rondeau began lecturing his philosophies in the mid-1980s. Dr. Rondeau's philosophy and lecturing style became so well-liked and well-known that the dental lab suggested he travel and lecture across North America. A few years later, the lectures became so successful that the dental lab could no longer manage the volume of registrations and inquires about Dr. Rondeau's lectures. In 1997, Rondeau Seminars Limited was created to promote and support Dr. Rondeau's seminars across the globe.

Along with lecturing and traveling, he has become one of North America's most sought after clinicians and has approximately 1,200 active patients in his office in London, Ontario. Over 18,000 dentists have attended his courses and study clubs in the U.S., Canada, Hong Kong, Australia, England and Poland.

Jim challenges the dental team to step outside its comfort zone to learn about the many populations with varied impairments in need of our services. With knowledge, compassion and a concerted team effort many special needs patients can be treated in the normal schedule of a dental practice.

Dr. James R. Schroeder served since 1994 through 2007 as the Midlothian representative on the Chesterfield County School Board.  He served as Chairman of the Board four of those years.  The five-member board provides leadership and governance for a student body of over 59,000, including 60 schools, 7,000 employees, and a budget of over $500 million dollars.

Dr. Schroeder is a founding member of the Crossover Health Ministry, in which both medical and dental volunteers provide care for the poor.  He was nationally recognized for improving access to care for the underprivileged and special needs populations. Valentine Richmond History Center recognized Dr. Schroeder as “Richmond History  Maker of 2009” for his innovative work in  starting Crossover Health Center.

Dr. Schroeder has been married to Jan, a former elementary school teacher, for 37 years. They are the proud parents of six children, ranging in age from 14 to 30, as well as three grandchildren. Biography:

Dr. Stuart was born in Illinois and moved to Southern California when he was five years old. He spent his childhood and teen age years doing the kinds of things that were done in LA in the 1950s and 60s. School, cars, surf, fun and sun. In late 1965 he was drafted into the United States Army and served a year of duty as an airborne combat medic in the jungles of Vietnam. During that time he had the extreme good fortune to meet and become friends with a medical doctor who saw potential in him and inspired him to do something meaningful with his life.

Upon his release from the military, he set his goals and began his college education at Santa Monica City College and then went on to the University of Southern California to finish his undergraduate work and obtain his doctorate degree from its school of dentistry.

In 1975, he, his wife and first child moved to Marin County California where he began his practice of general dentistry. During that time, he's been grateful that he's made the effort and had the good fortune to be a part of an evolution of modern dentistry that allows him to do things for his patients that he could have never dreamed of doing as a 21 year old sitting in a wet foxhole in Vietnam.

"Being able to provide relief for people suffering from head and neck pain, helping people with potentially life-threatening sleep breathing disorders, and rejuvenating unattractive smiles to the highest standards with the use of modern porcelain technology" -- are the three things that still keep Dr. Stuart most motivated and rewarded in the daily practice of his chosen profession.

    Biography:

" It’s deeply rewarding to provide real solutions to people who, oftentimes, have spent a great deal of time, money, and heartache trying to get help for their problem."

When I founded this practice in 1971, I helped people obtain healthier smiles. I also saw patients with problems that went well beyond what most general dentists (and many physicians, too) were able to treat. I knew I had to learn and do more!

For 30 + years, I’ve focused specifically on craniofacial pain, TMJ disorders and sleep disorders. In addition to ongoing research, I completed numerous fellowships and board certifications. To bring cutting-edge treatments to my patients, I also:

    • Served as a consultant to the Center for Devices and Radiological Health/Dental Products Panel of the Food & Drug Administration (FDA);
    • Founded the American Academy of Craniofacial Pain with my colleagues and served as this organization’s chairman of continuing education for the past 20 years; and,
    • Became the first (and only dentist) in Oklahoma to become a Diplomate in the Academy of Dental Sleep Medicine.
    • Much has changed since I graduated from dental school, yet much has remained the same. I’m still in Oklahoma where I was born and raised. I’m married to my college sweetheart, Pam Johnson, whom I married while we were attending The University of Oklahoma. And we’re still actively involved in our community and our church.
    • As a "dental physician," I am also still using a multidisciplinary approach to provide patients with conservative, non-surgical care. In addition to honesty, compassion and caring, experience has proven that this is best way to get the results you need…and deserve. For me, that’s what it’s all about
